| WBM-407 | Formal Request for Subsequent Transplant Peripheral Blood Lymphocyte collection | To request collection of Peripheral Blood Lymphocytes from a specific matched donor (for a second or subsequent transplant only). |
Conditions of Use of Service Requests Forms
- Service request forms are available in Adobe® .pdf format which may be downloaded and printed by use of the printer button in the Acrobat® toolbar. An Adobe® Acrobat® reader is required to access these forms, if not already installed a free copy can be downloaded by clicking on the "Get Acrobat® Reader" link below.
- Service request forms are strictly for Professional use only. Requests will only be actioned from centres recognised by WBMDR.
- All the information requested on each form must be filled in legibly and preferably in BLOCK CAPITALS to avoid any misunderstanding or confusion. If this is the first search request from a Transplant Centre then a contact telephone or Fax number must be provided for authentication, and further communications.
